Yellow Line Outcome Metrics for 2025
Goal 1: Improve participants timely engagement in behavioral health screenings.
- 1.A. Individuals involved with law enforcement officials who completed a Yellow Line screening.
- 1.B. Individuals screened as eligible for Yellow Line care coordination.
Goal 2: Improve participants connectivity to community-based services.
- 2.A. Individuals in contact with the Yellow Line Project provided Information and Referral materials.
- 2.B. Types of information and referral materials individuals requested and/or received.
- 2.C. Collaborative Outreach Team (COT) consultations and referrals to community-based services.
Goal 3: Reduce participants contact and/or involvement with law enforcement officials.
- 3.A. Individuals actively engaged in in Yellow Line care coordination services.
- 3.B. Individuals actively participating in Yellow Line care coordination services with no additional legal charges during care coordination engagement.
- 3.C. Individuals successfully engaged in in Yellow Line care coordination services and have no newly identified legal charges at 6 months and one year after case closure.
Goal 4: Improve community services providers engagement with the Yellow Line Project.
- 4.A. Collaborative engagements between the Yellow Line Project and community-based agencies, organizations, and service providers (quarterly report).
